SPONSORSHIP FORMS
Sponsorships Use this form to become a R4L financial sponsor. Become a Platinum Sponsor ($5000+), Gold Sponsor ($3500+), Silver Sponsor ($2500+), Bronze Sponsor ($1000+), Pink Ribbon Sponsor ($500+), or an Individual Sponsor ($100+).
Individual Award Sponsorship Use this form to sponsor a Breed Award, High Pledge Award, or a High Score Award (= a class award). $100 minimum.
Friends of the PVDA Ride for Life Use this form to be a sponsor for less than $100.
Class Check-Off Sheet Use this form to indicate High Pledge and High Score awards you want to sponsor.
RIDER PLEDGE/DONATION FORMS
Rider Donations Package Directions and all the forms needed by riders to submit Ride for Life donations. The package includes a Letter to Riders, a Rider Pledge Form, a Rider Pledge Transmittal Sheet (used to track multiple pledge checks for transmittal to rider donation committee; form allows you to fill in checks and then totals them for you), and a Rider Donation Combined Form (this form combines the most common donations that riders raise into one simple to use fill-in form; use one form per donor but donor may contribute in multiple areas).
Phantom Horse Class Entry Form People who will not be riding in the Ride for Life but would like to collect donations and compete for rider donation awards may enter the Phantom Horse Class. Use this form to enter the class.
FORMS FOR DONATIONS OF CLINICS AND OTHER ITEMS
Clinician Donation Use this form if you are a clinician donating a clinic for the Rider Raffle or the Silent Auction.
Rider Raffle Donations Use this form to submit non-clinic items that will be won by riders who bring in donations. Donate clinics using the Clinician Donation form.
Silent Auction Donations Use this form to submit non-clinic items for the Silent Auction. Donate clinics using the Clinician Donation form.
SATURDAY EVENING DINNER
Gala Dinner Use this form to order tickets for the Saturday evening Gala Dinner.
SHOW PROGRAM FORMS
